The Henry County Health Department will provide flu shots for all Galva students and staff on:

🏫 During the school day
⏰ Registration is due by October 1.
To register, please choose one of the following options:
✅ Complete the online registration form:

OR
✅ Request a paper consent form from Nurse Krystal by emailing kwest@galva224.org or sending a message through Rooms.
A completed registration or consent form is required to participate.
